Insurance Policies
If your insurance plan has a co-pay, you will be responsible for the co-pay for a specialist. If you have not reached your insurance plan’s deductible, you may also be responsible for additional costs associated with your visit.
For HMO plans that we accept, please contact your primary care physician (PCP) to obtain a Prior Authorization letter. Once you receive it, please call us to make an appointment.
For PPO plans that we accept, please call us directly to make an appointment.
